West Virginia coal mine accident

Three men working at a coal mine in West Virginia, slid into a slurry pond, which is a waste pit. Two of the men that slid into the pit were driving pick up trucks, made it out alive. While the other man, the was driving the bull dozer is still missing. Officials say that slurry ponds must be up to code and be inspected every month. But there was a failure somewhere and the family of the missing man is blaming the owner of the mine. While the mine owners say that the inspector is at fault, because they believe the inspector was careless. I personally believe it might be both of their faults.
